FORMS II Lite Software Current Users

The FORMS II Lite software is being used by Regional and State Agency users, as well as contractors in the following states:

There were 55 sampling organizations using the FORMS II Lite software for the Contract Laboratory Program (CLP) during Fiscal Year 2003. These organizations include 23 State sampling agencies and the US Army Corps of Engineers. In Fiscal Year 2003, FORMS II Lite was used to process 1,021 CLP Cases comprised of 3,485 shipments that contained a total of 51,145 samples.
