Overview
The Field Operations and Records Management (FORMS) II Lite system enables you to:  

  • Electronically create and edit both Contract Laboratory Program (CLP) and non-CLP sample documentation;

  • Electronically create and edit sample documentation in the field such as sample labels, sample tags, and Traffic Report/Chain of Custody (TR/COC) forms;

  • Reduce the margin of error when creating sample documentation by elimination of handwritten paperwork;

  • Reduce the field time necessary for completing complex sample collection activities and submitting the appropriate documentation;

  • Facilitate electronic capture of sample information into the user's FORMS II Lite database both prior to and during environmental field sampling operations;  and

  • Track samples from collection in the field to submission and analysis at the laboratories.

The FORMS II Lite software system is an Environmental Protection Agency Analytical Services Branch (EPA ASB) developed, stand-alone software program to support the mission of EPA and its contractors who visit hazardous waste sites to take samples of soil, water, air, and other sample matrices. It is a Windows-based application designed to automate the field sampling process.

Currently, all ten EPA Regions and several State agencies have implemented the use of FORMS II lite. EPA contractors usually take FORMS II Lite software into the field and EPA is responsible for oversight and control of the product's use.

FORMS II Lite software has evolved from Superfund project requirements; however EPA's objective has been to develop a generic software application that could easily be tailored to fit multiple sampling event activities. There are currently a number of custom features within the FORMS II Lite software that allow for adaptation to meet generic and non-CLP analysis needs.  For example, users could:

  • Add their own analytical method to the system and assign that method to a particular group of samples;

  • Add laboratories, matrices, preservatives, etc., to the reference tables in order to completely customize valid values;

  • Generate Sample Numbers based on the Station Location name, or enter a custom Sample Number; or

  • Use the Customize Field Labels feature to rename and customize the labels that automatically display within the Wizard interface.
